Plug-ins:  Gradient Glow

Drop Shadow used: Vertical 3, Horizontal 3, Opacity 82, blur 4, color: #202020

Font: Casper   (click name for link)

Remember, please use all images properly, and license's where applicable.

We will start from the bottom layer up, unless otherwise stated.

Abbreviations used:  
DS - drop shadow
RTF - resize to fit
C/P - copy and paste

Ready?  Let's begin!

Open 600 x 600

(please, don't forget to save as you go!)

Paste paper 2, RTF, and place just off center to the upper left.

Paste tube in image, RTF, add DS of choice, duplicate (we will use the copy soon)

C/P element 10, RTF, add DS, refer to my image for placement.

Paste frame 4, RTF, add DS of choice.  
Take your magic wand, and on the frame layer, click inside frame.
Expand selection by 7, invert, activate paper layer, delete; 
activate one of the tube layers, delete.
Select none.

Bring your duplicate tube above the frame layer. 
Erase parts that are not needed.
Refer to my image for guidance.

Open the following elements, C/P into the image.  
RTF, add DS of choice and refer to my image for placement:

13 (I used DS color #008000)
17 (I used DS color #008000)
100 (duplicated 9 times)
113
41
65 
66
65 (again)
117, no ds,
duplicate of 117 no ds, flip horizontal

Add copyright layer with proper license. 

Add name, convert to raster, inner bevel:  Bevel 2, width: 21, smoothness 94, depth 29, ambiance 0, shininess 30, Angle 4, Intensity 31, Elevation 63, color #ffffff
Then add Eye Candy Gradient Glow to name, width 12, Corners, 25, Opacity 100, color from tube add DS.
